Job Summary:
We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our Home Health team as a Support Staff member. The ideal candidate will provide essential care and support to patients in their homes, ensuring their comfort and well-being.
Responsibilities and Duties:
1. Assist patients with activities of daily living, including bathing, dressing, and feeding.
2. Monitor patient vital signs and report any changes or concerns to the nursing staff.
3. Administer medications as prescribed by a healthcare provider.
4. Assist with home exercise programs and mobility aids as needed.
5. Maintain a clean and safe environment for patients.
6. Provide emotional support and companionship to patients.
7. Document patient care activities accurately and timely.
8. Collaborate with the healthcare team to develop and implement individualized care plans.
Qualifications and Skills:
1. Active CNA certification in the state of Hawaii.
2. High school diploma or equivalent.
3. Previous experience in home health or a similar setting preferred.
4. Strong communication and interpersonal skills.
5. Ability to work independently and as part of a team.
6. Compassionate and patient-focused approach to care.
7. Basic computer skills for documentation purposes.
