Job Summary:
We are seeking a compassionate and dedicated Social Worker to join our team. The Social Worker will be responsible for providing support and assistance to individuals and families facing a variety of challenges. This individual will work closely with clients to assess their needs, develop treatment plans, and connect them with appropriate resources and services.
Responsibilities and Duties:
- Conduct assessments to determine clients' needs and develop individualized treatment plans
- Provide counseling and support to individuals and families facing social, emotional, and behavioral issues
- Connect clients with community resources, such as mental health services, housing assistance, and support groups
- Collaborate with other healthcare professionals, including doctors, nurses, and psychologists, to ensure comprehensive care for clients
- Monitor clients' progress and make adjustments to treatment plans as needed
- Advocate for clients' rights and assist them in navigating complex systems, such as healthcare and social services
Qualifications and Skills:
- Bachelor's or Master's degree in Social Work or related field
- Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) preferred
- Strong communication and interpersonal skills
- Ability to work effectively with individuals from diverse backgrounds
- Knowledge of social service systems and community resources
- Experience working with individuals and families in crisis
