Job Summary:
We are seeking a dedicated and compassionate Care Manager to provide personalized support and assistance to individuals in need of care. The Care Manager will be responsible for assessing the needs of clients, developing care plans, coordinating services, and ensuring the overall well-being of those under their care.
Responsibilities and Duties:
1. Conduct comprehensive assessments to identify individual needs and develop personalized care plans.
2. Coordinate and oversee the delivery of services such as medical care, home health aids, and therapy.
3. Monitor the progress and effectiveness of care plans, making adjustments as needed.
4. Advocate for clients and ensure they have access to necessary resources and services.
5. Collaborate with healthcare professionals, caregivers, and family members to provide holistic care.
6. Maintain accurate records and documentation of client interactions and services provided.
7. Stay current on best practices in care management and healthcare regulations.
Qualifications and Skills:
1. Bachelor's degree in social work, nursing, psychology, or a related field.
2. Previous experience in care management or a related field.
3. Strong communication and interpersonal skills.
4. Ability to work independently and as part of a team.
5. Knowledge of healthcare systems and community resources.
6. Excellent organizational and time management skills.
