Job Title: RN Home Health
Location: Kāne‘ohe, HI
Shift: 8hr days (40 guaranteed)
Duration: 13 weeks
Job Summary:
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our home health team. The ideal candidate will provide skilled nursing care to patients in the comfort of their own homes, ensuring they receive the highest quality of care possible. The RN Home Health will work closely with physicians and other healthcare professionals to develop and implement individualized care plans for each patient.
Responsibilities and Duties:
- Conduct initial patient assessments and develop comprehensive care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and assess patient progress, making adjustments to care plans as needed
- Educate patients and their families on disease management and self-care techniques
- Coordinate care with other healthcare providers and community resources
- Maintain accurate and timely documentation of patient assessments and care provided
- Adhere to all regulatory requirements and standards of practice
Qualifications and Skills:
- Current RN license in the state of practice
- Minimum of 1 year of nursing experience, preferably in home health or a related setting
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and collaboratively as part of a multidisciplinary team
- Reliable transportation and willingness to travel to patients' homes
- Basic computer skills for documentation and communication purposes
