Job Title: Care Manager
Job Summary:
As a Care Manager, you will be responsible for coordinating and managing healthcare services for individuals in need of assistance. You will work closely with clients, healthcare providers, and insurance companies to ensure that care plans are implemented effectively and efficiently.
Responsibilities and Duties:
- Assessing the needs of clients and developing individualized care plans
- Coordinating medical appointments and treatments
- Monitoring the progress of clients and making adjustments to care plans as needed
- Providing emotional support and guidance to clients and their families
- Collaborating with healthcare providers to ensure continuity of care
- Advocating for clients to ensure that they receive appropriate services and resources
- Documenting all interactions and maintaining accurate records
Qualifications and Skills:
- Bachelor's degree in nursing, social work, psychology, or a related field
- Previous experience in healthcare or social services is preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
