Job Title: Social Worker
Job Summary:
The Social Worker will be responsible for providing support and assistance to individuals and families facing a variety of social, emotional, and psychological challenges. They will work closely with clients to assess their needs, develop treatment plans, and connect them with resources and services to help improve their overall well-being.
Responsibilities and Duties:
1. Conduct assessments and interviews to determine the needs of clients
2. Develop individualized treatment plans and goals for clients
3. Provide counseling and support to clients to help them cope with their challenges
4. Connect clients with community resources, such as mental health facilities, housing assistance, and medical services
5. Collaborate with other professionals, such as psychologists and healthcare providers, to ensure clients receive comprehensive care
6. Monitor clients' progress and adjust treatment plans as needed
7. Maintain accurate and up-to-date records of client interactions and progress
Qualifications and Skills:
1. Bachelor's or Master's degree in Social Work or related field
2. State licensure as a Social Worker
3. Excellent communication and interpersonal skills
4. Ability to work effectively with individuals from diverse backgrounds
5. Strong problem-solving and decision-making skills
6. Knowledge of local resources and services for clients
7. Ability to work independently and as part of a team
