Job title: Care Manager
Job Summary:
The Care Manager is responsible for coordinating and managing the care of patients in a healthcare setting. They work closely with patients, families, and healthcare providers to ensure that the patient's needs are met and that they receive the highest quality of care possible.
Responsibilities and Duties:
- Develop and implement care plans for patients based on their individual needs
- Communicate with healthcare providers to coordinate and monitor patient care
- Advocate for patients and their families to ensure they receive appropriate care
- Monitor and evaluate patient progress and adjust care plans as needed
- Provide support and education to patients and their families about their care plan
- Collaborate with other members of the healthcare team to provide holistic care to patients
- Maintain accurate and up-to-date patient records
Qualifications and Skills:
- Bachelor's degree in nursing or a related field
- Licensed as a registered nurse or social worker
- Previous experience in care management or healthcare coordination
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of healthcare regulations and guidelines
- Excellent organizational skills
