Job Title: Long-Term Care Licensed Practical Nurse (LTC LPN)
Job Summary:
The LTC LPN is responsible for providing high-quality nursing care to elderly or chronically ill patients in a long-term care facility. They work under the supervision of registered nurses and physicians to ensure the well-being and comfort of residents.
Responsibilities and Duties:
- Administer medications and treatments as prescribed by healthcare providers
- Monitor patients' vital signs and condition, reporting any changes to the medical team
- Provide basic medical care, such as wound dressing and catheter care
- Assist residents with daily activities, such as bathing, dressing, and eating
- Collaborate with interdisciplinary team members to create and implement individualized care plans
- Educate patients and their families on managing health conditions and promoting wellness
- Maintain accurate and up-to-date medical records
Qualifications and Skills:
- Valid LPN license and certification in the state of practice
- Minimum of 1 year of experience in long-term care or geriatric nursing
- Strong communication and interpersonal skills
- Attention to detail and ability to multitask in a fast-paced environment
- Compassionate and patient-centered approach to care
- Ability to work effectively as part of a healthcare team
- Basic computer skills for medical record documentation
