Job: Medical Social Worker

Job Location: Gardner, MA 01440

Contract: 13 Weeks (Extension Available)

Shift: Day Shift

Guarantee: 40 hours a week

Pay Rate: $2000 - $2200/week

Duties:

• Provide emotional support and counseling to patients and families.
• Assist with discharge planning and coordination of post-hospital care.
• Help patients navigate healthcare systems and access resources.
• Advocate for patient needs and rights within the healthcare setting.
• Conduct psychosocial assessments and develop care plans.

Requirements:

Minimum Education

• Associates Degree required, Bachelor's Degree preferred in SW or related field

Minimum Work Experience

• Min. 1-3 yrs. experience in hospital setting, SNF, or health/community agency

Minimum Licenses and Certifications

• Social Workers MA State SW licensure-LCSW, LSW, LSWA and /or LICSW required
