Job Summary:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team in providing exceptional care to patients in the Long-Term Care (LTC) setting during night shifts. As an LTC LPN, you will play a vital role in the comprehensive healthcare of our residents, ensuring their comfort, safety, and well-being while adhering to all medical protocols and regulatory standards.
Responsibilities and Duties:
1. Provide direct nursing care to residents, following established care plans and medical protocols.
2. Administer medications, monitor vital signs, and conduct regular health assessments.
3. Collaborate with healthcare professionals to plan and implement individualized care plans.
4. Assist in the coordination of resident care, including medical appointments and treatments.
5. Ensure accurate and timely documentation of residents' medical records and progress.
6. Monitor and evaluate changes in residents' conditions and report any significant findings to the appropriate personnel.
7. Provide emotional support and maintain effective communication with residents and their families.
8. Adhere to infection control procedures and maintain a safe and clean environment for residents.
9. Demonstrate professionalism, compassion, and respect in all interactions with residents, families, and staff members.
10. Assist in the training and orientation of new LPNs and support staff as needed.
Qualifications and Skills:
1. Valid and active Licensed Practical Nurse (LPN) license in the state of [state].
2. Proven experience in long-term care or related healthcare settings preferred.
3. Strong knowledge of nursing principles, procedures, and medical protocols.
4. Excellent interpersonal and communication skills.
5. Ability to work independently as well as collaboratively in a team environment.

12. Willingness to work night shifts and weekends as required.
