Job Title: LPN, Long Term Care (LTC)
Job Summary:
As an LPN in Long Term Care (LTC), you will be responsible for providing exceptional nursing care to patients in a long-term care setting. You will work closely with other healthcare professionals to ensure the physical, mental, and emotional well-being of residents.
Responsibilities and Duties:
- Administer medications and treatments as prescribed by physicians
- Monitor and assess patient’s health status
- Collaborate with other healthcare team members to develop and implement care plans
- Provide emotional support and guidance to patients and their families
- Assist patients with activities of daily living
- Maintain accurate and updated patient records
- Follow all safety and infection control protocols
- Adhere to all state and federal regulations
Qualifications and Skills:
- Valid LPN license
- Previous experience in long term care preferred
- Strong communication and interpersonal skills
- Ability to work well both independently and as part of a team
- Compassionate and empathetic nature
- Attention to detail and strong organizational skills
- Ability to prioritize and multitask effectively
