Job Title: Social Worker - Clinical, Acute Care
Job Summary:
We are seeking a dedicated and compassionate Clinical Social Worker to join our team in the Acute Care setting. The ideal candidate will provide clinical social work services to patients and families in crisis, helping to improve their mental and emotional well-being during their hospital stay.
Responsibilities and Duties:
1. Conduct psychosocial assessments to identify and address patients' needs.
2. Provide crisis intervention and supportive counseling to patients and families.
3. Collaborate with interdisciplinary healthcare team members to develop and implement treatment plans.
4. Advocate for patients' rights and coordinate community resources for follow-up care.
5. Maintain accurate and timely documentation of patient interactions and progress.
Qualifications and Skills:
1. Master's degree in Social Work from an accredited program.
2. Licensed Clinical Social Worker (LCSW) in the state of practice.
3. Minimum of 2 years of clinical social work experience in an acute care setting.
4. Strong communication and interpersonal skills.
5. Ability to work effectively in a fast-paced and high-stress environment.
