Job Title: Social Worker
Job Summary:
We are seeking a compassionate and dedicated Social Worker to join our team and provide support to individuals and families facing challenging situations. The Social Worker will work closely with clients to assess their needs, develop care plans, and connect them with resources and services to improve their overall well-being.
Responsibilities and Duties:
- Conduct comprehensive assessments of clients' social, emotional, and financial needs
- Develop individualized care plans in collaboration with clients and other healthcare professionals
- Provide counseling and support to clients and their families
- Connect clients with community resources, such as mental health services, housing assistance, and support groups
- Advocate for clients and help them navigate complex systems, such as healthcare and social services
- Monitor and evaluate clients' progress and make adjustments to care plans as needed
Qualifications and Skills:
- Bachelor's or Master's degree in Social Work or related field
- State licensure as a Social Worker
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of community resources and social services
- Experience working with diverse populations and addressing complex needs
