Job Title: RN Case Manager
Job Summary:
We are seeking a dedicated RN Case Manager to join our healthcare team. The RN Case Manager will be responsible for coordinating patient care, developing care plans, and ensuring quality healthcare services for our patients. This role requires excellent communication skills, strong clinical knowledge, and the ability to collaborate with healthcare professionals to achieve optimal patient outcomes.
Responsibilities and Duties:
1. Collaborate with physicians, healthcare providers, and patients to develop comprehensive care plans.
2. Coordinate patient care services, including scheduling appointments, arranging for necessary treatments, and monitoring patient progress.
3. Provide patient education and support to promote self-management and improve health outcomes.
4. Conduct assessments and evaluations to determine patient needs and develop appropriate care plans.
5. Communicate with insurance providers and other stakeholders to ensure proper reimbursement for services rendered.
6. Maintain accurate and up-to-date patient records in compliance with healthcare regulations.
7. Participate in case conferences and collaborate with interdisciplinary teams to coordinate patient care.
8. Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
Qualifications and Skills:
1. Valid RN license in the state of practice.
2. Bachelor's degree in Nursing preferred.
3. Minimum of 2 years of experience in case management or a related field.
4. Strong clinical knowledge and expertise in patient care.
5. Excellent communication and interpersonal skills.
6. Ability to work independently and as part of a team.
7. Knowledge of healthcare regulations and insurance processes.
