Job Title: RN Case Manager
Job Summary:
The RN Case Manager will be responsible for coordinating and managing patient care throughout the healthcare continuum. They will work collaboratively with physicians, healthcare providers, and insurance companies to ensure optimal patient outcomes and cost-effective care.
Responsibilities and Duties:
1. Conduct comprehensive assessments of patients' health and social needs
2. Develop and implement individualized care plans in collaboration with the healthcare team
3. Coordinate and facilitate necessary medical services, therapies, and support services for patients
4. Monitor and evaluate patients' progress and adjust care plans as needed
5. Serve as a patient advocate and assist with navigating the healthcare system
6. Provide education and support to patients and their families
7. Collaborate with insurance companies to ensure coverage for necessary services
8. Maintain accurate and timely documentation of patient care activities
9. Participate in multidisciplinary team meetings to discuss patient progress and updates
10. Stay current on healthcare regulations and guidelines related to case management
Qualifications and Skills:
- Valid RN license
- Bachelor's degree in Nursing (BSN) preferred
- Minimum of 2 years of nursing experience, with at least 1 year in case management
- Strong communication and interpersonal skills
- Ability to prioritize and multitask in a fast-paced environment
- Knowledge of medical terminology, healthcare processes, and insurance procedures
- Proficiency in electronic medical records and other healthcare software
