Job Title: Long Term Care (LTC), LPN
Job Summary:
The Long Term Care (LTC), LPN is responsible for providing high-quality nursing care to patients residing in long-term care facilities. The LPN will work alongside a team of healthcare professionals to ensure the physical and emotional well-being of patients, while effectively communicating with patients' families and collaborating with other medical professionals.
Responsibilities and Duties:
1. Administer medications, treatments, and assist in rehabilitation exercises as prescribed by doctors.
2. Monitor and record patients' vital signs, symptoms, and changes in their physical or emotional conditions.
3. Assist in developing and implementing individualized care plans for patients.
4. Collaborate with other healthcare professionals, including doctors, registered nurses, and therapists, to provide comprehensive patient care.
5. Ensure patients' comfort, safety, and well-being by assisting with activities of daily living, such as bathing, dressing, and grooming.
6. Observe and report any significant changes in patients' conditions to the designated healthcare team members.
7. Educate patients and their families about healthcare procedures, medication management, and self-care techniques.
8. Maintain accurate and complete patient records, including medical histories, symptoms, and treatment plans.
9. Participate in multidisciplinary meetings to discuss patient progress and update care plans accordingly.
10. Adhere to all healthcare regulations, policies, and procedures to ensure patient safety and maintain a healthy work environment.
11. Assist in monitoring and ordering medical supplies, equipment, and medications as needed.
12. Provide emotional support to patients and their families, while fostering a compassionate and caring environment.
13. Collaborate with social workers and discharge planners to ensure successful transitions of patients from long-term care facilities to other settings, such as home care or rehabilitation centers.
14. Stay up-to-date with advances in nursing practices, technology, and applicable laws or regulations.
15. Continuously improve skills through professional development, training, and educational opportunities.
Qualifications and Skills:
1. Valid LPN license in the state of employment.
2. Proven experience as an LPN in a long-term care or similar setting.
3. Excellent knowledge of nursing care methods and procedures.
4. Ability to communicate effectively with patients, their families, and other healthcare professionals.
5. Strong organizational and time-management skills.
6. Ability to work independently and as part of a team in a fast-paced environment.
7. Compassionate and empathetic nature towards patients' physical and emotional well-being.
8. Proficient in accurately documenting patient data and maintaining confidentiality.
9. Familiarity with medical software and computer systems used for recordkeeping.
10. Physical ability to lift, push, and pull heavy objects and assist with patient transfers.
11. Knowledge of infection control protocols and procedures.
12. Flexibility to work day, night, and weekend shifts as required by the facility.
13. Attention to detail and ability to prioritize and manage multiple tasks efficiently.
14. CPR certification and other relevant certifications are desired.
15. Commitment to ethical and professional conduct in all job responsibilities.
