Job Title: LPN Long Term Care (LTC)
Job Summary:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team in providing exceptional care to residents in a Long Term Care (LTC) facility. As an LPN in LTC, you will work closely with a multidisciplinary team to ensure the physical and emotional well-being of our residents. Responsibilities and Duties:
- Administer medication and treatments as per physician's orders
- Monitor residents' health conditions and promptly report any changes to the registered nurse or supervisor
- Collaborate with the healthcare team to develop personalized care plans for residents
- Assist residents with activities of daily living, including bathing, dressing, and grooming
- Document residents' progress, observations, and interventions in accurate and timely manner
- Follow infection control procedures to maintain a safe and healthy environment
- Provide emotional support and comfort to residents and their families
- Participate in care conferences and contribute to care plan updates
- Collaborate with other healthcare professionals to ensure comprehensive care for residents
- Adhere to ethical and professional standards of nursing practice
- Maintain regulatory compliance and uphold facility policies and procedures
- Continuously enhance nursing skills and knowledge through ongoing education and professional development
Qualifications and Skills:
- Current and valid LPN license
- Graduation from an accredited nursing program
- Proven experience in a long-term care setting is highly desirable
- Sound knowledge of nursing principles and practices
- Strong communication and interpersonal skills
- Ability to effectively prioritize and multitask in a fast-paced environment
- Excellent critical thinking and problem-solving abilities
- Compassionate and patient-oriented approach towards residents and their families
- Proficient in electronic health record systems and basic computer skills
- Must be able to work flexible hours and adapt to changing resident needs
