Job Description:
We are seeking a passionate and skilled Physical Therapist (PT) to work in our team. The PT will be responsible for conducting evaluations and developing treatment plans for patients needing skilled services, such as physical therapy and rehabilitation. This role requires a compassionate professional dedicated to improving patients' quality of life through personalized therapy programs.

Key Responsibilities:

• Conduct comprehensive evaluations of patients to determine their physical therapy needs.
• Develop and implement individualized treatment plans aimed at achieving the highest functional outcomes for patients.
• Provide skilled physical therapy services, including but not limited to therapeutic exercises, manual therapy, and functional training.
• Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal results.
• Maintain accurate and timely documentation in compliance with regulatory and facility requirements.
• Participate in facility meetings, in-services, and continuing education opportunities to stay current with best practices.

Qualifications:

• Valid Physical Therapist (PT) license in the state of California.
• Graduation from an accredited physical therapy program.
• Strong clinical skills and a thorough understanding of rehabilitation practices in a skilled nursing facility preferred but not required..
• Excellent communication, interpersonal, and organizational skills.
• Ability to work independently and as part of a team.
• Prior experience in a skilled nursing facility is preferred but not required.
