Job Summary:
We are seeking a dedicated and compassionate Registered Nurse to join our team in providing high-quality care to patients in a Post Acute Long Term Care (LTC) setting. The ideal candidate will have experience in providing comprehensive nursing care to patients with acute and chronic conditions.
Responsibilities and Duties:
- Assessing patients' health status and developing individualized care plans
- Monitoring and recording patients' vital signs and symptoms
- Administering medications and treatments as prescribed
- Collaborating with healthcare providers to coordinate patient care
- Educating patients and families on disease management and treatment options
- Ensuring compliance with regulatory standards and best practices in healthcare
Qualifications and Skills:
- Current RN license in the state of practice
- Minimum of 1-3 year of nursing experience in a Long Term Care setting
- BLS certification
- Strong communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Excellent organizational and time management skills
