Job Title: Physical Therapist

Job Summary:

We are seeking a skilled and compassionate Physical Therapist to join our team. The ideal candidate will be responsible for evaluating and treating patients with physical impairments to help them improve movement, manage pain, and prevent or recover from injuries. As a Physical Therapist, you will work closely with patients to create personalized treatment plans and provide hands-on care to help them achieve their goals.

Responsibilities and Duties:

- Evaluate patients' medical history and perform physical assessments to determine diagnoses

- Develop and implement individualized treatment plans to address patients' specific needs

- Provide hands-on therapy, exercises, and techniques to help patients improve mobility, strength, and range of motion

- Educate patients and their families on at-home exercises and self-care techniques

- Monitor patients' progress and adjust treatment plans as needed

- Collaborate with other healthcare professionals to ensure comprehensive patient care

- Maintain accurate and up-to-date patient records

REQUIRED:

• Bachelor’s degree from the Commission of Accreditation in Physical Therapy Education (CAPTE) accredited school of Physical Therapy. 
• Must possess a valid, current, full and unrestricted license in any state within the United States, District of Columbia, or the Commonwealth of Puerto Rico during tenure of employment.
•  Must have and maintain a current Basic Life Support (BLS) certification by the American Heart Association. 
• Applicant must meet requirements to be credentialed to serve on the Medical Staff.  
• All related information required for credentialing must be provided.
