Job Title: Physical Therapist (PT) - Long Term Care (LTC)
Job Summary:
We are seeking a dedicated and compassionate Physical Therapist to join our Long Term Care facility team. The PT will assess and treat residents with physical limitations due to injury, illness, or aging, to improve their mobility and quality of life. The ideal candidate will have a strong focus on patient-centered care and be able to collaborate effectively with interdisciplinary team members.
Responsibilities and Duties:
1. Conduct comprehensive evaluations to determine residents' physical therapy needs.
2. Develop personalized treatment plans and goals for each resident.
3. Provide hands-on therapy interventions, exercises, and modalities.
4. Educate residents and their families on safe techniques and exercises to improve function.
5. Monitor and document progress in residents' functional abilities.
6. Coordinate care with other healthcare professionals to ensure comprehensive treatment plans.
7. Maintain professional standards and adhere to all ethical and legal requirements.
Qualifications and Skills:
1. Graduate of an accredited Physical Therapy program.
2. Current PT license in the state of practice.
3. Previous experience in Long Term Care or geriatric rehabilitation preferred.
4. Strong communication and interpersonal skills.
5. Ability to work independently and as part of a team.
6. Familiarity with electronic medical records and documentation.
7. CPR certification.
8. Strong clinical reasoning and problem-solving abilities.
