Job title: Licensed Practical Nurse (LPN) - Long Term Care (LTC)
Job Summary:
As an LPN working in Long Term Care facilities, you will provide direct patient care and support to residents with chronic conditions or disabilities. Your role will involve administering medications, monitoring vital signs, collaborating with healthcare team members, and ensuring the overall well-being of residents in the facility.
Responsibilities and Duties:
1. Administer medications as prescribed by healthcare providers
2. Monitor and record vital signs and symptoms of patients
3. Assist residents with activities of daily living, such as bathing and feeding
4. Collaborate with physicians, registered nurses, and other healthcare professionals to develop and implement care plans
5. Provide emotional support and companionship to residents
6. Ensure the safety and comfort of residents at all times
7. Document patient care activities and communicate effectively with the healthcare team
8. Educate patients and families on health management and disease prevention strategies
9. Adhere to all facility policies and procedures to maintain a safe and clean environment
10. Participate in staff meetings and training sessions to enhance skills and knowledge
Qualifications and Skills:
1. Active LPN license of IL
2. Graduation from an accredited nursing program
3. Previous 2+ experience working in Long Term Care settings preferred
4. Strong communication and interpersonal skills
5. Ability to prioritize and multitask in a fast-paced environment
6. Compassionate and patient-centered approach to care
7. Knowledge of nursing best practices and standards
8. Proficiency in electronic medical records systems
9. Ability to work independently and as part of a team
10. Current BLS certification
11. Strong attention to detail and organizational skills
12. Ability to remain calm and focused in stressful situations
13. Flexibility to work different shifts, including nights and weekends
14. Commitment to ongoing professional development and continuing education
15. Physical stamina to stand, walk, and lift patients as needed
