Job Title: RN Case Manager
Job Summary:
The RN Case Manager is responsible for coordinating the care and services for patients in a healthcare setting. They work closely with healthcare providers, patients, and their families to ensure that the best possible care is provided.
Responsibilities and Duties:
- Assessing patients' needs and developing care plans
- Coordinating and scheduling appointments and services
- Monitoring patients' progress and adjusting care plans as needed
- Providing education and support to patients and their families
- Collaborating with healthcare providers to ensure coordinated care
- Advocating for patients' needs and rights
- Documenting and updating patient records
Qualifications and Skills:
- Current RN license
- Bachelor's degree in Nursing (BSN) preferred
- Minimum of 2 years of nursing experience
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of healthcare laws and regulations
- Experience with electronic health records (EHR) systems
