Job Title: RN Case Manager - Hospice
Job Summary:
We are seeking a compassionate and experienced Registered Nurse to join our hospice team as a Case Manager. The RN Case Manager will be responsible for coordinating and overseeing the care of hospice patients, ensuring that they receive the highest quality of care and support during their end-of-life journey.
Responsibilities and Duties:
- Conduct initial assessments of patients to determine hospice eligibility
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Coordinate with physicians, caregivers, and other healthcare professionals to ensure holistic care for patients
- Provide direct patient care including medication administration, wound care, and symptom management
- Educate patients and families on end-of-life care and support services available
- Monitor patient progress and adjust care plans as needed
- Maintain accurate and timely documentation of patient care and progress
Qualifications and Skills:
- Current RN license in the state of practice
- Minimum of 2 years of experience in hospice or home health care
- BSN preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate and empathetic attitude towards patients and families
