Job Title: Licensed Practical Nurse (LPN) - Long-Term Care (LTC)
Job Summary:
As an LPN in a Long-Term Care facility, you will be responsible for providing comprehensive nursing care to residents under the direction of the Registered Nurse. Your primary goal will be to ensure the health and well-being of residents while promoting a comfortable and supportive environment.
Responsibilities and Duties:
1. Administer medications and treatments as prescribed by healthcare providers.
2. Monitor residents' health status and update care plans as needed.
3. Assist with personal care activities, such as bathing, grooming, and dressing.
4. Collaborate with the interdisciplinary team to develop and implement individualized care plans.
5. Document resident care and observations accurately and in a timely manner.
6. Respond to medical emergencies and provide immediate care as needed.
7. Educate residents and families on health maintenance and disease prevention.
8. Adhere to all state and federal regulations related to long-term care facilities.
Qualifications and Skills:
1. Current LPN license in the state of employment.
2. Minimum of 1 year of experience in a long-term care setting.
3. Excellent communication and interpersonal skills.
4. Ability to prioritize tasks and work efficiently in a fast-paced environment.
5. Strong clinical assessment and critical thinking skills.
6. Basic computer skills for electronic charting and communication.
7. CPR certification.
8. Compassionate and patient-centered care philosophy.
